What would cause a push rod to wear in rings like this?

Maybe little dents in the tubes causing rubs?

Believe they are supposed to rotate but not aware of any contact areas.

Damaged (squashed or dented) pushrod tube or as vwkj87 says the pushrod tube retaining clip is broken or out of place.

The tips were chewed up a bit when the rocker assembly nuts fell off.

That would also result in the push rods wobbling around

Lucky the rods are in one piece!
Impressed that the engine was still going with nuts falling off!

Oh, they look very similar to mine that I just removed ... and neither head had the retaining springs
New heads have been purchased (for other reasons) but I presume that they will be without the springs too.
I also have one slightly damaged tip ... can I replace just one pushrod (cut to same length as the one being replaced) or do all need doing at the same time?

If you buy a complete Head, they should come with it.
